  • Informed consent for any kind of treatment and research protocol.
  • Information on diet & nutrition.
  • Information about immunization.
  • Confidentiality.
  • Access clinical records.
  • Get the information on plan of care.

PATIENT RESPONSIBILITIES
  • To provide complete & accurate information.
  • To follow the prescribed treatment plan & instructions carefully.
  • Not to take any medication without the knowledge of doctor.
  • To accept the measures taken by the hospital for maintaining privacy & confidentiality of medical records.
  • Responsibility for safe cutody of personal belongings & valuables.
  • To pay the bill on time.
  • To treat hospital staff, other patients & other visitors with respect.
  • To help us to keep the hospital surrounding clean & not to damage the hospital property.
  • Strict proibition for smoking / usage of tobacco in any form.
  • To maintain silence as much as possible.
  • To abide by all hospital rules & regulations.
  • To co-operate with hospital staff during treatment.
  • To meet the treating doctor daily to know about progress of patient.
